You do not need to use a specific form or wording to make a Subject Access Request. Requests can be made verbally or in writing. 

When making a request, please provide as much information as possible to help us identify you and locate the information you are requesting. 

We may need to ask you to provide information to confirm your identity before we can provide personal information. 

What happens after I make a request? 

Once we receive your request, we will review it and contact you if we need any further information to process your request or verify your identity. 

We will normally respond to a Subject Access Request within one month of receiving the request and any information we reasonably require verifying your identity. 

In certain circumstances, where a request is particularly complex or where you have made multiple requests, the response period may be extended where permitted by data protection legislation. If this applies, we will let you know.
